While there's details in the article about why, the one thing lawyers and courts wish more clinical teams would remember is how in clinical notes and records, every word matters. Easier said than done when of course, admin is one of the major time strains and reasons for clinical burnout... however, the words that are used when charting and the outlook that's shared when tweaking macros and adjusting them as time permits can often cost and create vastly different outcomes for patients. Paradoxical that the clinical team is there to really help in a practical sense and don't like to chart... but it's the chart that so often is the best way to help the patient when they aren't in front of the clinicians.
